About South Tyneside IRT Project

South Tyneside's Identification, Referral and Tracking (IRT) Project was established in 2003, with funding provided by the Department for Education and Skills (DfES)

The aim of the IRT project is to prevent social exclusion among children and young people, by improving the exchange of information between agencies that work with them.

In order to achieve this aim, the project has a number of objectives, including:

  • To develop a system to identify children and young people (aged 0 –19), who are at risk of different forms of social exclusion. The system will enable the development of co-ordinated support for those identified as being 'at risk'
  • Developing a directory of all services which are available to children, young people and their families in South Tyneside
  • Developing integrated procedures to enable professionals from different agencies to work with 'at risk' young people in a consistent and co-ordinated way
